A Clinical Evaluation of Spontaneous Pneumothorax

Abstract
In this study, 101 cases of spontaneous pneumothorax experienced at the Department of Thoracic Surgery, SNUH during the period from January, 1960 till December, 1972, were reviewed.
1) Among the 101 cases composed of 79 male patients and 22 female patients, the male to female ratio was found to be 3.2:1, showing male preponderance.
2) The incidence according to the age groups showed that 70% of the patients belong to the age group from 3rd to 5th decade.
3) The frequent chief complaints were as follows; sudden dyspnea in 88%, chest pain in 51% and coughing in 17%, respectively.
4) The etiologic factors were as fowilows; tuLerculous origin in 55. 4%, acute pulmonary infection in 7.9%, rupture of bullae or blebs in 7.9%, pulmonary paragonimiasis in 4% and unknown cause in 24.7%, respectively.
5) The study on the predilection site showed involvement of left side in 51. 5%, right side in
43.6% and bilateral in 4. 9%, respectively.
6) The degree of collapse of the lung was variable but in 53.4% of the patients, the collapsed area was more than 50% of the ipsilateral lung field and collapse over 30% of the lung field was observed in 79% of the patients.
7) The emlpoyed methods of treatment were as follows.
In 4 patients, successful expansion could be obtained with bed rest only. Needle aspiration was performed in 14 cases, of which 8 cases showed complete expansion but in 6 cases additional closed thoracotomy drainage was necessary. Closed thoracotomy performed in 88 cases showed successful expansion in 71 cases but in the remaining 17 cases thoracotomy was needed.
